Terms & Conditions for Healthcare Provider Registration · Praxis Connect Health Partners Private Limited · Version 1.0
These Terms & Conditions ("Terms") govern the registration and use of the Praxis Connect platform ("Platform," "we," "us," or "our") by hospitals, clinics, nursing homes, and individual medical practitioners (each a "Healthcare Provider," "you," or "your") who register on the Platform. By creating an account, checking the acceptance box, and completing registration, you agree to be bound by these Terms. If you do not agree, you must not register on or use the Platform.
"Platform" means the Praxis Connect website, mobile application, and related services operated by Praxis Connect Health Partners Private Limited, a company incorporated under the laws of India.
"Healthcare Provider" means any hospital, clinic, nursing home, diagnostic centre, or individually registered medical practitioner that creates an account on the Platform.
"Referral" means the act, facilitated through the Platform, of one Healthcare Provider directing a patient to another Healthcare Provider for treatment, diagnosis, or follow-up care.
"Patient Data" means any personal data, sensitive personal data, or health data relating to an identified or identifiable patient that is entered into, uploaded to, or shared through the Platform.
"Applicable Law" includes, without limitation, the Digital Personal Data Protection Act, 2023 ("DPDP Act") and rules made thereunder, the National Medical Commission Act, 2019 and regulations issued by the National Medical Commission ("NMC") or applicable State Medical Council, the Clinical Establishments (Registration and Regulation) Act, 2010 (where applicable in your state), the Information Technology Act, 2000 and rules made thereunder, and the Consumer Protection Act, 2019.
I confirm that I have read and agree to the Praxis Connect Health Partners Pvt Ltd Terms & Conditions and Privacy Policy. I certify that all information I have provided, including my medical registration / establishment licence details, is accurate and complete, and I agree to keep it up to date. I understand that I am solely responsible for the accuracy of this information and for any claim, loss, or regulatory action arising from inaccurate or misleading information I provide. I understand that Praxis Connect may suspend or terminate my account, with or without notice, in case of any violation of these Terms.
